How to change the profile without sacrificing the tooth show?

This young man complained about protrusion and malalignment after some relapse of prior orthodontic treatment.

He already had four premolars extracted.

With a pretty acceptable tooth show, we reduced the vertical dimension by double arch intrusive distalization instead of retraction only.

Profile had some improvement because of the counterclockwise rotation of the mandible, soft tissue strain had improved.

He still has a nice and broad smile after the retreatment.
